Hello,

Thank you for enrolling yourself or your child into 2023 dance classes with Richmond Indian Dance Academy, LLC (RIDA). We are very excited to be a part of your dance journey! Please read and acknowledge the statements below PRIOR to the first class.

GENERAL

• Fees will be collected prior to the first class of the session. Fees must be paid on time to keep dancer’s place in a class, regardless of their absences.

• If a client chooses to not continue classes for whatever reason, fees will not be refunded or waived.

  
  

RULES

• Dancers need to be dropped off no earlier than 5 minutes before their scheduled classes. Parents are welcome to stay during the duration of the class in the sitting area, however we ask that you please stay quiet and that there is no distraction or interaction with the students during the class.

• Dancers need to be picked up on time.

• There will be no guaranteed supervision other than when the dancers are in the classroom

  with their teachers.

• All dancers must be potty trained.

• Siblings are not allowed to wait unsupervised.

• Parents are responsible for their children’s messes and behavior while in the studio.

• NO FOOD, DRINK (other than water, sealed in a tight-sealed bottle), or GUM.

• If your dancer will be at the studio for MORE than three hours, they may bring a granola bar or something similar to keep in their bag. NO CHIPS, CANDY, MESSY SNACKS, ETC.

• All dancers are advised to bring a filled water bottle to class and to wear comfortable, loose-fitting clothes to dance. No jeans.

I agree to abide by the above fees collection and make up class guidelines. I also agree to myself/my child abiding by the above stated rules before, during and after classes.

In recognition of the above rules and acknowledgement, I knowingly and voluntarily waive all right and/or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to RIDA, its officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as RIDA). I hereby agree to release RIDA and hold RIDA harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance on behalf of the participant. I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation. I also give RIDA permission to use my picture in or on any form of advertisement for RIDA or a RIDA affiliated event. If I am a minor, my parent(s) and/or legal guardians has/have also signed this document releasing RIDA from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risks of injury inherent to this activity. The participant has my permission to participate in RIDA Events. I acknowledge that RIDA and their directors, officers, volunteers, representatives and agents are not responsible for errors, omissions, acts or failures to act of any party or entity conducting a specific event or activity on behalf of RIDA. I further release RIDA of all liabilities associated with mine or my child’s attendance at RIDA. I acknowledge that I have carefully read this “waiver and release” and fully understand that it is a release of liability. I expressly agree to release and discharge RIDA and all of its affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns, from any and all claims or causes of action and I agree to voluntarily give up or waive any right that I otherwise have to bring a legal action against RIDA for personal injury or property damage.

Photo Release

I hereby grant permission for RIDA to take photos and/or videos of my child and/or myself for public and studio use including, but not limited to, advertising, marketing, education, etc.

I HEREBY AFFIRM THAT I HAVE READ AND FULLY UNDERSTAND THE ABOVE STATEMENTS. BY REGISTERING AND PAYING FOR ANY CLASS, EVENT, PRIVATE LESSON, OR OTHER ACTIVITY, YOU ARE AGREEING TO ALL OF THE ABOVE CONDITIONS AND WAIVERS. PLEASE DO NOT HAVE YOUR CHILD/YOURSELF PARTICIPATE IN ANY CLASS OR EVENT UNLESS YOU ACCEPT AND AGREE TO ALL OF THE TERMS IN THIS AGREEMENT.
